Southern Cross Care QLD has an exciting opportunity for a proactive and self-directed full-time Care Partner to join our Community Team working from our Edens Landing Community Care office.

What are the key responsibilities? 

As a Care Partner, you will work collaboratively with our elderly clients, who live in the Logan region area to engage in services that enable them to remain living in their home.

Based at our Home Care Offices, in this dynamic role you’ll manage a case load of Home Care Package (HCP) and Commonwealth Home Support Program (CHSP) clients, driving towards exceptional care outcomes. Your expertise will ensure that each client’s needs are met with top-quality service and support. You’ll be the key player in coordinating care strategies and using your in-depth knowledge and skills to make a real difference in our clients’ lives. Key responsibilities will include:

• Assessing the needs and preferences of each client in a caseload
• Developing personalised care plans in collaboration with clients and their families
• Coordinating the delivery of services such as personal care, meal preparation, cleaning, and transportation
• Monitoring the implementation of care plans and adjusting as needed
• Ensure CHSP services are provided in accordance with CHSP guidelines
• Advocating for clients' rights and preferences
• Conducting regular reviews and reassessments as needed to ensure that clients' needs are being met effectively
• Providing information and support to clients and their families regarding available services and resources
• Liaising with healthcare professionals, service providers, and community organisations to ensure coordinated care

What will you bring to the role?

• Experience in a similar role within Aged Care or Disability Services Sector
• Qualifications in Nursing or Allied Health (desired not essential)
• Demonstrated knowledge and experience in delivering services under Home Care Package Program
• Demonstrated knowledge and experience in delivering services under the Commonwealth Home Support Program (CHSP)
• Demonstrated knowledge and experience of working in accordance with Aged Care Standards and legislation
• Ability to manage time efficiently and prioritise responsibilities
• Excellent communication skills. Ability to communicate clearly and effectively with team members and clients
• Exceptional interpersonal skills and the ability to relate to a variety of people
