Southern Cross Care QLD is seeking an experienced part-time Nurse Practitioner, within the Aged Care environment, to join our team.
In this exciting position, our Nurse Practitioners work within a blended model encompassing on-site visits and tele-health covering approximately 1000 residents. Within your scope of practice as the Nurse Practitioner, you will be enhancing health care delivery and strengthening the capacity to manage residents by supporting the doctors working in General Practice and our nursing teams to provide optimal care to our residents within our Residential Aged Care Homes. Working in this role, you will be working along-side two other part time Nurse Practitioners and reporting to the Head of Clinical Practice, who is also a Nurse Practitioner, so you would not be alone.

What are the responsibilities?
In this role you would provide best practice clinical care to our residents in our homes within the scope of practice, including but not limited to the following:
- Providing skilled, holistic health assessment, history taking, diagnosis and treatment services to residents and clients.
- Ordering and/or performing diagnostic tests as needed, checking, interpreting and informing residents and clients of results in a timely manner, and follow up on appropriate course of action.
- Prescribing or de-prescribing of medications as per all relevant regulations and standards.
- Ensuring residents are appropriately followed up to ensure positive health outcomes.
- Referring patients appropriately to other providers if their needs exceed the Nurse Practitioner scope of practice you are able to provide.
- Consulting and collaborating with colleagues to provide optimal care.
- Documenting all care provided and education/information given to patients within their health record, as per professional and company standards.
- Supporting residents and clients in fulfilling their end-of-life wishes.
- Providing clinical leadership that influences and progresses clinical care and collaboration through all levels of health service (ANMC. 2006).
- Entering into and maintaining a collaborative arrangement with the relevant medical practitioners
- Ensuring ethical billing practices to assist the Nurse Practitioner business unit in achieving and maintaining strong financial health to meet or exceed Key Performance Indicators.
- Maintaining professional knowledge and standards through continuing education.
- Reporting as required by legislation when you believe patients are at significant risk.
- Maintaining trusting professional relationships with residents and clients and their chosen representatives
- Complying with AHPRA Good medical practice: a code of conduct for doctors in Australia, and the Australian Medical Association Code of Ethics.
- Providing services courteously and respectfully, with regard to the cultural beliefs and needs of residents and clients.
- Responding openly to complaints or feedback.
- Working constructively and harmoniously with other members of the health care team.
- Leading or delegating appropriately within the team.
- Training/Education of NP Candidates, nursing and care staff, students and residents.
What will you bring to the role? - Master’s degree in Nurse Practitioner Studies with APHRA Registration as Nurse Practitioner
- Primary care practice software experience
- Nurse Practitioner Provider Number and Prescriber Number
- Broad scope of practice is desired, including but not limited to diagnosis of acute health conditions in adults, Chronic Disease management and Palliative Care
- Ability to assess, investigate and implement appropriate treatments within scope of practice
- Experience in using Medicare billing software
- Demonstrated ability in effective communication and negotiation skills, including proven experience in mentoring and educating staff in formal and informal settings
- Strong commitment to evidence-based care and continuous quality improvement processes
- Demonstrated experience of influencing, interpersonal, consultation, relationship building and negotiation skills that build and sustain productive relationships and partnerships
- Understanding the aged care and acute care eco-systems, including the ability to work harmoniously and supportively with teams and families to provide evidence-based care to residents in our homes.
- Professional Liability Insurance as legally required for all practicing Nurse Practitioners
- Ability to travel periodically to our sites as well as at short notice when required
