About the role
As a Care Partner for the Western Downs, you will work collaboratively with our elderly clients, who live around Miles, Tara, Chinchilla, Meandarra and Jandowae area to engage in services that enable them to remain living in their home.
Based at our Home Care Office in Chinchilla or one of our other local offices, in this dynamic role you’ll manage a case load of Support at Home and Commonwealth Home Support Program (CHSP) clients, driving towards exceptional care outcomes. You’ll be the key player in coordinating care services and using your in-depth knowledge and skills to make a real difference in our clients’ lives. Your expertise and support will ensure that each client feels ‘Known and Loved’ by our service. Key responsibilities will include but not be limited to:
- Developing personalised care plans in accordance with assessed needs and clients wishes (and their families where appropriate).
- Developing budgets with clients, utilising funding to meet their needs effectively.
- Coordinating the delivery of services such as personal care, meal preparation, cleaning, and transportation.
- Monitoring the implementation of care plans and adjusting as needed
- Monitoring of clients funding, taking corrective action to ensure clients services are provided within funding and support the management of any unspent funds.
- Managing purchasing of clients’ consumables, ensuring goods are supplied as required.
- Ensure CHSP services are provided in accordance with CHSP guidelines
- Promoting clients' rights, preferences and independent decision making.
- Role modeling professional behaviours, and Southern Cross Care’s ‘Known and Loved’ framework, when working with clients and by being helpful and supportive to our Support Workers.
- Conducting regular reviews and reassessments as needed to ensure that clients' needs are being met effectively.
- Providing information and support to clients and their families regarding available services and resources.
- Liaising with healthcare professionals, service providers, and community organisations to ensure coordinated care.
- Maintaining accurate records of Care management services, and document client interactions and service provision.
- Participating in the out-of-hours ‘on-call’, on a rostered basis.
What will you bring to the role? - Experience in a similar role within Aged Care or Disability Services Sector
- Qualifications in Nursing or Allied Health (desired not essential)
- Ability to adapt and respond effectively to changing situations and needs
- Practical and solution focused approach to problems with a realistic and sensible mindset.
- Great organisation and time management skills with ability to priotise responsibilities.
- Well-developed attention to details, ensuring accuracy and thoroughness.
- Ability to follow processes and create and implement efficient work practices.
- Reliability: demonstrated ability to consistently meeting deadlines and commitments, fostering trust among team members and clients.
- Excellent communication skills. Ability to communicate clearly and effectively with team members and clients
- Exceptional interpersonal skills and the ability to relate to a variety of people
