The Position
Our innovative service model focuses on delivering the best clinical outcomes for clients. As a District Nurse with Empower, you will be responsible for the delivery of Empower Healthcare's comprehensive services across home care clients in the Eastern suburbs of Melbourne. You will:
- Conduct comprehensive clinical assessments and develop clinically appropriate care plans
- Provide a range of nursing services to clients within the community, including but not limited to Medication Management, Wound Care, Diabetes Management, Catheter Care, Stomal Therapy and Cognitive Assessments
- Work holistically with other healthcare professionals to provide a multidisciplinary approach to client care
- Educate clients and their families to optimise the client's ability to live safely within the community
- Work collaboratively with Case Managers and NDIS Support Coordinators to provide positive client outcomes
- Work autonomously to prioritise and complete tasks and complete all required documentation to a quality standard
Do you have what it takes?We are looking for passionate people who care about making a difference in the lives of our people. You will have an enthusiastic and proactive attitude, be outcome focused and dedicated to delivering exceptional service to our clients. You must have:
- Unrestricted Registration with AHPRA as a Registered Nurse
- Availability Mondays, Wednesdays and Fridays
- Sound clinical knowledge and skills
- Experience working with clients who have chronic and complex health needs
- Experience working with clients from diverse cultural and socio-economic backgrounds
- Excellent verbal and written communication skills
- Excellent time-management skills and attention to detail
- National Police Check Clearance
- Working with Children Check and NDIS worker screening
- Current CPR Certificate
- Availability across Mondays, Wednesdays and Fridays highly desirable
