Physical Therapist - Home Health (Part-Time/Day Shift).
We are seeking a Physical Therapist - Home Health who will provide home health physical therapy within the Physical Therapy Professional Scope of Practice, to restore function and prevent disability following disease or injury; assist the patient in reaching maximum ability within the limits of his/her capabilities and the disease process.
In this position you will:
Evaluation of Patients: Complete evaluation of patient in compliance with agency policy, law and regulation and within the Physical Therapy Professional Scope of Practice. Assist the physician in evaluating level of function and in developing the plan of care. Evaluate the home environment and makes appropriate recommendations
Treatment of Patients: Carriy out patient care in compliance with agency policy, law and regulation, and within the Physical Therapy Professional Scope of Practice as ordered by the attending physician, dentist or podiatrist
Reassessment of Patients:
Complete reassessment of patient in compliance with agency policy, law and regulation and within the Physical Therapy Professional Scope of Practice. Assist the physician in updating the plan of treatment
Provides Continuity of Care: Ensure appropriate planning for patient discharge and referral to other services according to agency policy and in compliance with law and regulation. Advise, consult and, when appropriate, instruct family and other agency personnel in patient’s therapy program
Communicates:
Communicate with Rehabilitation Coordinator and other health care professionals caring for the patient to ensure uninterrupted patient care. Observe records and report information on the patient’s condition to the attending physician and in the patient’s health record
Coordinates: Attend patient care conferences to ensure coordination of the patients care with other disciplines caring for the patient
Leadership:
Supervise Home Health Aide when assigned
Documentation/Information Management:
Document all patient care according to agency policy
Ensure confidentiality of all patient information
Development:
Assume responsibility and accountability for own professional growth and practice. This includes maintaining current licensure and completing all required in-service education
Attend all assigned staff development activities for department including a minimum attendance of 75% at all scheduled work time staff meetings
Participate in/conducts in-service education, as requested
Performance Improvement:
Participate as requested in agency Improving Organizational Performance activities
Promote use of the agency C.A.R.E. methodology in problem solving

Qualifications:
Degree in Physical Therapy from an approved program according to Medicare guidelines
Current California Physical Therapy license
BLS certificate
Requires a valid driver’s license with no excessive violations or accidents
Requires maintaining the minimum state requirements for auto insurance
2 years experience in acute care, rehabilitation, or home care
