Responsible for assisting and consulting with the Executive Director to provide leadership, direction, growth, outreach, and administration of Hospice operations and to ensure accomplishment of strategic objectives. Responsible for directing, supervising and coordinating activities of various clinical Hospice patient care services. Serves as a member of the Executive Team.

Responsible for the overall planning, delivery, and evaluation of quality end-of-life hospice nursing care for patients in their own home (single-family dwelling, retirement community, or other facility serving as the patient’s home) or hospice inpatient facility.
Coordinates the services, supervision and evaluation of Clinical Supervisors, Clinical Educator/Infection Control, other RNs/LPNs not assigned to a team, Social Work Supervisor, and Intake/Triage of the agency’s Hospice programs as appropriate and in accordance with approved care plans, policies and procedures, and applicable rules and regulations [WAC 248-31-125(3)].
In coordination with the Social Work and Supervisors, provide psycho-social and spiritual care services to ensure patients receive holistic, patient- and family-centered care.
In coordination with the Hospice Liaison’s conduct end-of-life care presentations in the community and in clinical settings. Promote agency hospice services at community events. Serve as hospice liaison in area hospitals and clinical facilities when needed.
Manage compliance with documentation processes to ensure that the clinical records meet legal, regulatory, payer and agency requirements.
All agency employees will follow the minimum standard regarding PHI when accessing patient information on a need to know basis only. All employees will practice appropriate safeguards when accessing this information and approve and monitor subordinate PHI systems access.
Essential Duties and Responsibilities
Manage quality and safety of patient care through oversight of hospice patient care nursing and ancillary clinical staff.
Manage administration, planning, oversight and evaluation of assigned programmatic areas.
Manage day-to-day operations of assigned programmatic areas through delegation of responsibility to supervisory personnel.
Provide oversight of interdisciplinary teams for quality and safe patient care and regulatory compliance.
Work closely with Executive Director to accomplish the following: respond positively to future challenges and opportunities with the goals of the highest quality of patient care, plus growth of the organization in terms of patient access and related services; identify and resolve operational and regulatory issues while keeping the Executive Director fully informed of activities, decisions and plans, and compile, prepare and submit reports as required or requested; and actively promote and engage in community activities to grow hospice services.
Work in collaboration with other members of leadership to further the overall goals of the organization. Is a leadership member of the Hospice Management Team, the Leadership Team, and the Executive Team, and attends and participates in these meetings.
Ensure safe and quality patient care through the effective communication, collaboration, and liaison with the Hospice Medical Director, patients’ primary physicians, and other health care facilities, agencies and professionals who provide services to Hospice patients. [WAC 248-31-125(4)]
Assist in the direction and coordination of services between nursing, therapies, pharmacies, other pertinent vendors, and clinical support staff.
Oversee all purchasing activities related to medical supplies/equipment and pharmaceuticals.
In collaboration with the Clinical Educator, identify and schedule in-services needed to keep staff knowledgeable about care for the terminally ill.
Participate in the on-call nursing supervisor rotation schedule. Follow through and complete the on-call supervisor duties. Is prepared and ready and able to step into direct patient care as needed.
Ensure that the agency is maintaining an adequate inventory of medical supplies and pharmaceuticals including the ordering, maintenance, and accounting of all items in accordance with hospice protocol.
Ensure Clinical Supervisors are fulfilling their assigned job tasks including but not limited to the need for all RNs and NACs document their activities in the patient’s clinical record in accordance with established record keeping procedures and requirements; and conduct ongoing Performance Improvement monitoring of patient clinical records.
Provide and document orientation, instruction, and regular supervision (including periodic direct observation) and feedback to all direct reports.
Work closely with Volunteer Coordinator to meet hospice program objectives including superior patient and family support.
Coordination and development of clinical component of patient care policies and procedures.
Provide leadership and formal/informal education to staff through in-services, orientation of new staff, annual reviews, care conferences and group discussions on hospice philosophy, operations, policy and procedures, pain and symptom management, and problem solving.
Conduct nursing staff meetings to report, reinforce, develop, discuss and or inform nursing teams of policy and procedure changes, process review or changes, QAPI projects, education.
Support a culture of safety and quality by participating actively in the Quality Assessment Performance Improvement (QAPI) committee by meeting attendance and other activities as needed (i.e. planning, chart reviews, reporting, and participating in Performance Improvement Projects (PIPs)).
Work with Human Resources to recruit and retain competent nursing staff. Assist Clinical Supervisors in interviewing and selecting RNs and NACs who provide direct patient care services in both hospice programs.
Supervise direct reports, including but not limited to mentoring, recruiting, training, and conducting performance reviews
When requested, provide feedback to the Executive Director to evaluate the performance of other staff who the Clinical Director collaborates with.
Maintain and enhance knowledge and skills in hospice nursing care by attending educational workshops, participating in professional organizations, and through individual research and reading.
Participate in the development and revision of written patient care and administrative policies and procedures related to each service provided.
Review all nursing policies and procedures at least annually.
Prepare and recommend revisions as needed to ensure that policies and procedures accurately reflect current practice and standards of care. [WAC 248-31-125 (2)]
Maintain knowledge of: the duties and responsibilities of all interdisciplinary team embers; community resources and services available to patients; participate in the coordination of interdisciplinary and interagency services [WAC 248-31-125(4)]; current applicable regulations, agency, and program policies and procedures pertaining to: patient care, safety, patient rights, professional practices, continuing quality improvement, record keeping, and other related subjects; accurately explain and interpret requirements to team members; and monitor compliance.
Participate in the planning, operation, and evaluation of clinical services.
Ensure that all members of the nursing staff are sufficiently trained regarding procedures and requirements to provide backup to, and act when requested, in the absence of the In-Home Hospice Clinical Nursing Supervisor or Team Leaders. [WAC 248-31-125(1)]
Participate in budget development and adheres to the board approved budget model. Monitors compliance of expenditures.
Responsible for monitoring of clinical budgets.
Provide utilization review regarding patient care expenditures and provides the billing department with payment authorization.
Work with Executive Director and other leadership to determine staffing needs and staffing patterns according to program requirements and with consideration for utilization of personnel at their maximum potential.
Assist with new program development.
Assume responsibility for agency administration in the absence of the Executive Director.
Receives, investigates, follows up and documents response to clinical occurrence/ incident reports, patient, family, health care provider complaints or grievances, negative family survey responses.
Responsible for activating the Emergency Preparedness Plan or assigning designee in Executive Director absence.
Participate in community education and public relations programs as requested by the Executive Director.

Direct Reports – In Home Hospice and Hospice House Clinical Supervisors; Infection Control/Clinical Educator; Intake/Triage; and Social Work Lead and co-report on Lead Supervisor

Current Washington State Registered Nurse’s License.
Graduate of an accredited school of nursing; Bachelor of Science degree in Nursing, MSN, MHA, or MBA preferred. Qualified RN may substitute four years of suitable management/supervisory experience and four years of clinical experience in lieu of educational requirements.
Minimum of two years clinical experience (hospice, home health preferred) with a minimum of two years management/supervisory experience.
Certification in Hospice and Palliative Care Nursing preferred or must be attained within three years of employment.
Experience in program development, coordination and management.
Experience in oncology, home health, or hospice nursing preferred.
Access to transportation.
Valid driver’s license and automobile liability insurance for your state of residency.
Tuberculin skin test by Mantoux Method, or chest x-ray (if previous Mantoux positive).
