We are looking to recruit an experienced Care Manager, or someone currently operating at a strong Deputy level, who will be responsible for leading the provision of care within an extra care service that also offers a small outreach service across Solihull.

As a Care Manager you will be responsible for the day to day reglatory compliance of the service. The most important responsibilities include ensuring that customers are receiving the care and support they expect and deserve, ensuring that Care staff are carrying out their duties to the highest standard and working closely with the care delivery team.


Key Responsibilities:

• Arranging initial assessment with the prospective customers (and/or their chosen representatives) to assess their needs and what they would like to achieve from their care and support.
• Conduct Risk Assessments and complete regular reviews and updates.
• Create and agree written individually tailored care plans. Agree appropriate risk control measures to reduce identified risks. Keep care and support plans up to date.
• Ensure the accuracy and completeness of all documentation held in the customer’s home.
• Liaise closely with the Care Coordinator regarding recruitment and matching of care staff to the customers.
• Carry out introductory visits to introduce care staff and report changes to care schedules to the coordinator, customer, and carer.
• Support, supervise and assist with training of care staff.
• Investigate matters relating to the quality of the service and implement improvements.
• Monitor the performance of Care staff and carry out quality reviews at the customer’s home to make sure the service is being delivered in line with company procedures.
• Work flexibly to meet business needs.
• Take part in the out of hours emergency on-call rota as and when required.

Personal Specification

• Extensive elderly care experience with a proven track record in providing consistent excellent customer service.
• Ideally held CQC Registration or willing to in next role
• Experience in leading, training and managing a care team.
• Have or be willing to work towards Level 3 Diploma in Leaderships for Health and Social Care as a minimum
• Strong skills in conducting care assessments and care planning.
• A full UK driving licence and your own car.
