Community Care Manager - Aged Care

The Role

As Community Care Manager, you will lead an established Support at Home service, supporting your team to deliver high-quality, person-centred care to community clients.

Key responsibilities include:

• Leading and supporting Care Partners and care staff
• Overseeing care plans, budgets and service reviews
• Supporting complex clients and escalations
• Monitoring compliance, quality, documentation and risk
• Managing incidents, complaints and performance matters
• Driving continuous improvement and staff development
• Working closely with clients, families and broader leadership teams

About You You may already be a Care Manager, or an experienced Senior Care Partner / Team Leader ready to step into a broader management role.

You will ideally have:

• Strong Home Care / Support at Home experience
• Experience leading or mentoring community care teams
• Strong knowledge of Support at Home and the Aged Care Quality Standards
• Experience with care plans, budgets and client reviews
• Strong understanding of quality, compliance and risk
• Relevant Community Services, Aged Care or Nursing qualification
