Your Role

As a Clinical Care Coordinator, you’ll take a hands-on leadership role in clinical care delivery while supporting and mentoring the nursing and support teams 

• Manage care planning and assessments 
• Lead a team of RN's, EN's and Support staff 
• Managing complex wound care and coordinating allied health referrals
• Oversee quality and carry out education for clinical staff. 
• Partner with families and allied health teams to deliver holistic care
• Foster a safe, supportive, and resident-focused workplace culture

About You

• Bachelor of Nursing with current AHPRA registration
• 1-2 years experience working as a Senior Nurse/Clinical Coordinator
• Proven ability to provide strong clinical leadership, mentoring, and support to nursing and care teams
• Sound knowledge of accreditation standards, compliance requirements, and care planning processes
• A genuine commitment to delivering high-quality, resident-centred care
