Community Aged Care Leader | NFP Benefits 

This is a well-supported community role offering flexibility, autonomy and the chance to build strong relationships within the local community. 

You’ll provide leadership to a team of Case Managers and Support Workers. You will ensure high quality care is being delivered and coordinate services based on client needs so they can continue to live safely and independently at home.

About the Role

• Provide leadership to a small and stable team 
• Drive growth within your portfolio and build on new and existing stakeholder relationships
• Manage services, referrals and oversee new client commencements
• Manage Support at Home funding and budgets across an active community portfolio
• Complete reviews and goal-based care planning
• Drive team culture and wellbeing 

What We’re Looking For

• Minimum 2 years’ experience in community aged care (Care Coordinator or Community Manager roles)
• Experience growing a community portfolio (desirable) 
• Excellent organisation and prioritisation ability
• Proactive, client-first approach
• Knowledge of aged care compliance, quality standards and care planning
