Care Coordinator - Support at Home (Community Aged Care)

Join a well-established community aged care provider delivering Support at Home services across North Brisbane.

This is a well-supported Care Coordinator role where you’ll manage your own caseload, with strong clinical, admin and leadership support in place.

It’s a great opportunity to step into a stable team, with realistic expectations and the ability to build proper relationships with your clients.

The role

• Manage a portfolio of community aged care clients under Support at Home
• Complete assessments, care planning and regular reviews
• Coordinate services in line with client goals and care plans
• Monitor budgets and funding utilisation
• Build strong relationships with clients, families and stakeholders
• Ensure compliance with Aged Care Quality Standards
• Work closely with clinical and scheduling teams

About you

• Experience in Care Coordination / Case Management / Community Aged Care
• Understanding of Support at Home, HCP or CHSP
• Confident managing a caseload and working autonomously
• Strong communication and stakeholder skills
• Organised, proactive and genuinely client-focused
