Community Registered Nurse - Aged Care

The Role: 

This role focuses on delivering high-quality nursing care to clients in their homes, supporting their independence and wellbeing. You’ll complete assessments, reviews, and clinical interventions while working closely with care staff to ensure clients receive timely, appropriate support.

Key Responsibilities

• Undertake nursing assessments, reviews, and care planning
• Provide community-based clinical care (including wound care, medication support, and chronic disease management)
• Work collaboratively with care staff to coordinate client needs
• Keep documentation accurate and up to date
• Offer guidance to ENs or less experienced nurses when required
• Participate in team meetings, discussions, and education sessions
• Ensure compliance with relevant aged care and community care standards
• Build respectful, trusting relationships with clients and families

What You’ll Bring

• Current AHPRA RN registration
• Minimum 2 years experience as a Registered Nurse within Community Aged Care
• Solid experience in clinical assessments, wound care, and chronic disease management
• Ability to guide or support ENs or junior nurses
• Understanding of aged care community care frameworks
• Valid driver’s licence
