Home Care Case Manager – Community Aged Care

Key Responsibilities:

• Develop and manage individualised care plans in consultation with clients, families, and key health professionals
• Conduct assessments and regular reviews to ensure client needs and goals are being met
• Coordinate community-based services in line with Aged Care Quality Standards and program guidelines
• Maintain accurate documentation, monitor client budgets, and ensure compliance with the Support at Home framework
• Build strong, trusted relationships with clients, families, and multidisciplinary teams
• Provide information, advocacy, and education to help clients make informed decisions about their care

Requirements:

• Previous experience in Care Coordination / Case Management / Home Care 1-2 years minimum 
• Solid understanding of HCP, CHSP and Aged Care Quality Standards
• Strong relationship-building, communication, and organisational skills
• Qualification in Aged Care, Community Services, or similar (Cert III/IV or Diploma) - preferred not essential
• Current Driver’s Licence, NDIS Worker Screening, and First Aid/CPR
