Case Manager – Community & Home Care | South Brisbane

A fantastic opportunity to join a leading community care provider delivering NDIS and Aged Care services across South Brisbane. You’ll coordinate tailored care for a balanced caseload of clients while working in a supportive, values-driven team that genuinely cares.

Ideal for an experienced Case Manager or Registered Nurse seeking stability, flexibility and a workplace that puts people first.

Key Responsibilities

• Deliver holistic case management from intake through to care planning, implementation, and review
• Develop and manage individual care plans and budgets in consultation with clients and families
• Conduct clinical reviews and provide mentoring and guidance to support staff
• Build trusted relationships with clients, carers, and external networks
• Travel locally to visit clients across Brisbane South

Requirements:

• Nursing qualification and current AHPRA registration (preferred, not essential)
• Strong understanding of NDIS, DVA and/or Home Care Package frameworks
• Excellent communication, coordination, and problem-solving skills
• A compassionate, proactive approach with genuine care for client outcomes
• Current Driver’s Licence and reliable vehicle
