About the Role
In this role, you will deliver culturally safe, trauma-informed assertive outreach services, aligned with the contractual requirements and activity schedule.
The role is responsible for delivering early intervention, relapse prevention, and case management, including care planning and coordination, to Aboriginal individuals experiencing mental health (MH) and/or alcohol and other drug (AOD) challenges.
The position provides proactive, place-based outreach and works in partnership with individuals, families, Elders, and service providers to:
- improve access to services,
- support recovery and wellbeing,
- strengthen continuity of care, and
- address service gaps within the community.
The role operates within a culturally responsive, community-led framework and contributes to improved outcomes for individuals with complex and co-occurring needs.
Key features of the role include but are not limited to:
Organisational / Program Delivery - Deliver early intervention services, including screening and brief intervention
- Provide relapse prevention and post-treatment support
- Deliver case management, care planning and coordination aligned to individual needs
- Provide assertive outreach services
- Facilitate access to MH and AOD services
Community Development & Cultural Responsiveness Partnerships & System Integration - Develop and maintain referral pathways with MH, AOD and broader services.
- Establish and maintain formal partnerships and service agreements.
- Participate in shared care planning and coordinated service delivery.
- Work collaboratively across primary through to tertiary care and community-based organisations

Suitable candidates will possess the following:
Qualifications & Experience - Applicable Certificate IV qualification or equivalent
- Experience:
- delivering assertive outreach services, and/or case management
- working with Aboriginal and Torres Strait Islander individuals, families and communities
- with early and brief intervention approaches
- with relapse prevention and recovery-oriented practice
- engaging individuals who are hard to reach or not currently accessing services
- Experience working within:
- multidisciplinary environments
- community-based or outreach service models
Skills & Capability - Highly organised with strong time management and prioritisation skills
- Ability to manage multiple competing priorities in a fast-paced environment
- Strong written and verbal communication skills
- High attention to detail and accuracy
- Ability to maintain confidentiality and handle sensitive information
- Strong systems thinking and ability to improve processes

Additional requirements:
- Working With Children Check
- National Police Clearance
- Hold a current and valid Western Australian C class motor driver’s licence or equivalent
- Australian Citizenship or permanent resident of Australia
