We are seeking an experienced Social Worker (Grade 2) to join our team in a full time capacity! This role is based at Nhill but may be required to work across all sites as mutually agreed upon.

As the Social Worker (Grade 2), you will work with individuals, families, groups and communities to assist with, and improve well-being through casework, counselling, advocacy and community development.

The role involves working with acute inpatients, outpatients and aged care residents and includes psycho-social assessment, discharge planning, education and referral.

The Grade 2 Practitioner may also have responsibility for the supervision of students.


KEY SELECTION CRITERIA
Essential:

• Tertiary qualification in Social Work and eligibility for AASW membership.
• At least two years’ experience working the Social Work field.
• Proficient clinical skills in psychosocial assessment, risk assessment, complex care planning, social work interventions and discharge planning.
• Demonstrated understanding of theoretical frameworks which inform Social Work practice.
• Effective communication and organisational skills

PRE-REQUISITES:

• NDIS Worker Screening Clearance
• Working with Children Check
• Fully vaccinated (including influenza) as per current Department of Health Directives.
• Australian Driver’s License
