Join our Community Health team as the CHSP Care Partner!

Seeking Certificate III individual support (or equivalent) qualified staff with a passion to support clients over 65 living at home.
  
We are looking for WWHS staff that wish to extend their part time hours for a minimum of 8 hours a fortnight (up to 2 days per week) to become a CHSP Care Partner.
  
Reporting to the CHSP coordinator, CHSP Care Partner’s role will be to meet CHSP clients and develop care plans.

ABOUT THE ROLE:

• The primary purpose of this position is to coordinate and ensure the effective delivery of services to WWHS Commonwealth Home Support Program (CHSP) participants who have been assessed through the Single Assessment System and funded to implement a support plan.

KEY RESPONSIBILTIES:

• Develop a care plan for WWHS CHSP participants consistent with their aged care assessment.
• Work with the participant to determine how those services will be delivered, involving registered supporters, informal carers and family members in the discussions with consent of the participant.
• Forward the care plan and service delivery choices to the CHSP Services Coordinator who will develop the CHSP Agreement and coordinate the provision of services for each participant as per the care plan, including referral to external services for assessment and services.
• Perform a review of the care plan when needs change and in line with the strengthened Quality Standards. To be part of the community care rotating after-hours on-call phone roster.
