We are seeking a Social Worker (Grade 1 or Grade 2) to join our team, based at Nhill (but may be required to work across all sites as mutually agreed upon).
Full Time - 80hrs per Fortnight, including Accrued Days Off (ADO) (or Part Time Hours Negotiable)

Classification: AHP1 - Grade 1, Year 2 - Grade 2, Year 4 (Level will be dependent on experience as per Award).
ABOUT THE ROLE:
The Grade 1 Social Worker role focuses predominantly on Social Work service delivery working with individuals, families, groups and communities to assist with, and improve well-being through casework, counselling, advocacy and community development. The role involves working with acute inpatients, outpatients and aged care residents and includes psycho-social assessment, discharge planning, education and referral.
The Grade 2 Social Worker is an experienced clinician who works with individuals, families, groups and communities to assist with, and improve well-being through casework, counselling, advocacy and community development. The role involves working with acute inpatients, outpatients and aged care residents and includes psychosocial assessment, discharge planning, education and referral. The Grade 2 Practitioner may also have responsibility for the supervision of students.
KEY SELECTION CRITERIA: Essential:
Grade 1:
Tertiary qualification in Social Work and eligibility for AASW membership.
Graduate level clinical skills in psychosocial assessment, risk assessment, care planning, social work interventions and discharge planning.
Demonstrated understanding of theoretical frameworks which inform Social Work practice.
Effective communication and organisational skills.
Grade 2, as above and:
At least two years’ experience working the Social Work field.
Proficient clinical skills in psychosocial assessment, risk assessment, complex care planning, social work interventions and discharge planning.
Desirable:
Grade 1 and Grade 2:
Previous experience working in a hospital/healthcare setting.
Experience and understanding of issues specific to provision of services in rural and remote areas.
Competence in using computer programs including Microsoft Word, Excel, Outlook and other databases such as iCare, isoft and Uniti.
Experience utilising the My Aged Care (MAC) portal.
Knowledge and experience undertaking goal directed care planning.
