We are currently looking for a Nurse Practitioner (Primary Care) to fill in a position for a Nurse Practitioner-led clinic. 

This is a permanent hire position located two (2) hours from Toronto, Ontario. Relocation will be expected and required from the successful applicant. Applicants must also be willing to start ASAP. 

The Role: As a Nurse Practitioner (NP), you will provide comprehensive, patient-centered care by diagnosing and managing acute and chronic medical conditions, prescribing medications, and ordering diagnostic tests. You will work independently and in collaboration with physicians and healthcare teams, with a focus on patient education, preventive care, and treatment across a broad spectrum of health conditions.

Responsibilities:

• Conduct comprehensive patient assessments, taking detailed medical histories and performing physical examinations.
• Diagnose and manage a variety of acute and chronic medical conditions.

• Order and interpret laboratory tests, X-rays, and other diagnostic procedures.
• Prescribe medications and administer treatments as necessary.
• Educate patients and their families on health care, disease prevention, and health promotion strategies.
• Collaborate with an interdisciplinary team (e.g., physicians, therapists, nutritionists) to develop and implement patient care plans.
• Advocate for patient care needs and support patients in navigating the healthcare system.

• Coordinate and manage primary patient care independently or as part of a collaborative team.

Requirements:

• Completion of a Nurse Practitioner program and relevant training.
• Registration with the College of Nursing in Ontario as a Nurse Practitioner
• New graduates or nurse practitioners without experience are welcome to apply

Compensation: 

• Salary offer is: $125,000 to $130,000 per year
