As a Licensed Practical Nurse (LPN) in Designated Supportive Living (DSL), you will provide compassionate, client-centered care to residents in a supportive living environment. Your primary focus will be fostering independence, dignity, and quality of life while working collaboratively with a multidisciplinary team to deliver personalized care services tailored to residents' unique needs.
Responsibilities:
- Provide direct nursing care, including administering medications, managing chronic conditions, and performing wound care.
- Monitor residents' health and respond to changes in their condition by reporting concerns to the appropriate healthcare team members.
- Support residents with activities of daily living (ADLs) while promoting their autonomy and dignity.
- Develop, implement, and regularly update individualized care plans in coordination with residents, families, and other healthcare professionals.
- Ensure accurate and timely documentation of care, assessments, and interventions following regulatory standards and organizational policies.
- Maintain a safe, clean, and supportive environment that prioritizes resident comfort and well-being.
Requirements: - 2 years LTC experience is required within the last 5 years.
- 2 years acute care (med/surg) experience is required within the last 5 years.
- Current and active BLS-HCP is required.
- No pet friendly or family friendly accommodations available.
- Must be available until September 30, 2025
- Charge experience is preferred and must be clearly listed on resume.
- Connect Care trained (Rural Nurse module) is preferred.
- Previous experience with AHS (within the last 2 years) as a LPN in the same unit/site is preferred.
