As an LPN in LTC, you will play a vital role in providing quality patient care and supporting healthcare professionals in delivering exceptional services to patients.
Responsibilities:
- Conduct assessments, collect data, and provide basic nursing care to patients under the direction of a registered nurse or physician.
- Administer medications and treatments as prescribed by healthcare providers.
- Monitor and document patient vital signs, symptoms, and responses to treatment.
- Assist with patient education and support, including managing patient and family expectations and concerns.
- Maintain accurate and up-to-date medical records and documentation.
- Participate in interdisciplinary team meetings and contribute to care planning and coordination efforts.
- Adhere to established policies, procedures, and standards of nursing practice.
Requirements: - Bachelor’s degree in nursing.
- Current, unrestricted LPN license in your province.
- 1 year LTC experience is required within the last 5 years.
- Current and active BLS-HCP is required.
- Connect Care trained is required.
