As an LPN in the Med/Surg, ER, and Long-Term Care (LTC) units, you will deliver comprehensive patient care in various settings, ensuring the highest standards of nursing practices. You will be responsible for assessing, planning, implementing, and evaluating care for patients with acute medical conditions as well as those requiring ongoing support in long-term care.
Responsibilities:
- Conduct thorough assessments to determine patient needs and prioritize care accordingly.
- Administer medications and treatments as per physician orders and established protocols.
- Monitor and document vital signs, lab results, and patient responses, ensuring accuracy and timely reporting.
- Collaborate with interdisciplinary teams to develop and implement individualized care plans for patients in acute and long-term care settings.
- Provide education and support to patients and their families regarding care plans, treatment options, and discharge instructions.
- Uphold safety standards by maintaining a clean and sterile environment, adhering to infection control practices and safety protocols.
Requirements: - Completion of a recognized Practical Nursing program.
- Current, unrestricted LPN license in your province.
- Minimum 2 years of acute care (med/surg).
- Current and active BLS-HCP and CLPNA licensure is required.
- Current and active CTAS, ACLS is preferred.
