Bundaleer Home Care, part of Apollo Care, is seeking a dedicated Clinical Care Partner to join our growing team. This is an exciting newly created opportunity to provide person-centred clinical care that helps older Australians remain independent and continue living safely in their own homes.
  
About the Role
As a Clinical Care Partner, you will work closely with clients, families and the wider care team to deliver high-quality clinical support across a range of home care programs. No two days are the same, with responsibilities including clinical assessments, care planning, nursing interventions and supporting clients to achieve their goals.
  
This role offers the perfect balance of autonomy, meaningful client relationships and a supportive team environment.
  
Key Responsibilities

• Conduct comprehensive clinical assessments and reviews
• Develop and maintain person-centred care plans
• Deliver nursing interventions within your scope of practice
• Support clients with wound care, medication management, chronic disease management, diabetes care and other clinical needs
• Monitor client wellbeing and identify clinical risks
• Coordinate referrals to GPs, allied health and specialist services
• Provide clinical guidance and support to Care Partners and Support Workers
• Maintain accurate documentation and compliance with funding requirements

About You

To be successful in this role, you will have:

• Current AHPRA registration as a Registered Nurse
• Current First Aid and CPR certification
• Current driver's licence and access to a reliable insured vehicle
• NDIS Worker Screening Check (or willingness to obtain)
• At least 1 year of experience in community nursing, home care or a similar clinical setting
• Strong assessment, clinical decision-making and communication skills
• A genuine passion for person-centred care
