Are you a dedicated Care Manager with a passion for leading care teams and supporting older Australians to live with dignity and grace? PresCare Yaralla Place, part of the Apollo Care Alliance, is seeking a compassionate and experienced Care Manager to join our team.

About the Role: As a Care Manager, you will play a key leadership role in ensuring the delivery of safe, high-quality, resident-centred care in our lovely 96 bed home. Reporting to the Residential Service Manager, you will guide and support our care team, ensuring compliance with the Aged Care Quality Standards and Apollo Care’s model of care.
  
Key Responsibilities:

• Lead and support the care team to deliver holistic, person-centred care.
• Oversee care planning, clinical governance, and resident assessments.
• Ensure compliance with regulatory requirements, including the Aged Care Act 1997.
• Promote resident choice, family engagement, and a strong sense of home.
• Drive continuous improvement in quality, safety, and resident experience.

About You:

• Current registration with AHPRA as a Registered Nurse
• Minimum 5 years’ experience as a Clinical Nurse, Care Coordinator, or Care Manager (with at least 2 years in aged care)
• Strong leadership and team management skills
• Excellent knowledge of the Strengthened Aged Care Quality Standards and clinical governance.
• A genuine commitment to supporting residents to live their way with dignity and grace
• Strong verbal and written communication skills
• Compassionate and Empathetic nature
