The Role:
We are looking for a Care Planner who are passionate about a person-centred approach to care, to join our wonderful facility in Yackandandah in the North East region of Victoria.
The Care Planner promotes safe, high-quality care and accurate recurrent funding for residents by ensuring care plans are accurate and up to date. The Care Planner is responsible for assessment, care planning and care evaluation in accordance with company and resident policies and works closely with the Care Manager and Care Team to achieve their objectives.
