National Health Recruitment is looking for a dedicated and experienced Support Coordinator / Psychosocial Recovery Coach. This role combines coordination and coaching to provide comprehensive support to individuals on their psychosocial recovery journey, helping them improve well-being, develop coping strategies, and achieve personal goals.
As a Support Coordinator / Psychosocial Recovery Coach, your responsibilities include:
Client Assessment and Planning:
- Conduct comprehensive assessments of clients' psychosocial needs, strengths, and challenges.
- Work closely with clients to create personalized recovery plans aligned with their NDIS goals and aspirations.
- Regularly monitor and evaluate progress, making adjustments to plans as needed.
- Participate in NDIS plan re-assessments, reviews, and AAT tribunal meetings.
- Oversee and assess clients' NDIS plans, ensuring alignment with their needs.
- Strong budgeting and numerical skills are essential.
Coordination and Resource Referral: - Collaborate with external service providers, healthcare professionals, and community resources to ensure comprehensive support for clients.
- Assist clients in accessing relevant social services, housing, employment, and other essential resources.
- Coordinate both mainstream and non-mainstream support services.
Psychosocial Support and Coaching: - Offer empathetic, non-judgmental coaching using evidence-based techniques and interventions.
- Provide emotional support, active listening, and guidance to individuals facing mental health challenges.
- Assist clients in developing coping strategies and building resilience.
- Create and submit a Recovery Plan and Report to the NDIS.
Documentation, Reporting, and Administration: - Keep accurate client records, progress notes, and intervention documentation.
- Prepare regular reports on client progress for stakeholders.
- Stay informed on best practices, research, and trends in psychosocial recovery and the NDIS.
- Participate in training and workshops to enhance coaching skills.
- Meet daily and weekly KPI targets.
To succeed in the role, you will need: - Diploma in Community Services, Mental Health, Alcohol & Other Drugs, or Youth Work, and other related diploma.
- Relevant certifications or training in Social Services.
- At least 1 year of experience in Support Coordination and/or Psychosocial Recovery Coaching.
- Strong interpersonal skills, with the ability to build rapport, trust, and maintain boundaries.
- Excellent verbal and written communication skills.
- Knowledge of NDIS, community resources, and social services.
- Ability to work effectively with a team.
- Proficient in documentation and record-keeping.
- Ethical and culturally sensitive approach to client care.
