We are currently recruiting for a Social Worker / Support Coordinator for our client, a values-led organisation providing tailored, high-impact support for people living with disabilities and psychosocial challenges. This is a rare opportunity to take on a role that combines therapeutic practice with the strategic coordination of NDIS plans, all while enjoying the freedom to manage your caseload and make a meaningful difference.

Your Key Responsibilities
- Create, review and implement individualised support plans for both social work and support coordination strategies aligned with NDIS goals
- Coordinate services, monitor progress and respond to any issues or changes
- Collaborate effectively within a multidisciplinary team to ensure a holistic and consistent approach to client care
- Provide emotional support and advocacy for individuals facing challenges such as disability, mental health issues, trauma, addiction or life transitions
- Provide hands-on support, advocacy and crisis response where required
- Conduct comprehensive psychosocial assessments to understand client needs and risks
- Build strong relationships with clients, families, service providers, and community organisations.
- Educate clients about their NDIS plan, funding and support options
- Maintain clear, up-to-date case notes and documentation in line with compliance

What You Bring - A recognised Social Work degree and eligibility for AASW membership
- Minimum three years of experience in social work, case management or NDIS support
- Ability to work independently and as part of a multidisciplinary team
- Strong working knowledge of the NDIS and person-centred approaches
- Excellent communication, advocacy and organisational skills with
- A confident and compassionate approach to client care
- Valid driver’s licence and access to a reliable vehicle
