Manager – Care Planning & Practice x2  | Gold Coast & Southwest Brisbane Region 

NDIS | Child Safety | Disability Services | Leadership Role

Lead care delivery. Manage complex clients. Drive compliance and team performance.

We’re looking for an experienced Care Planning Manager / Practice Leader to oversee care planning, compliance, and frontline teams across Child Safety and Disability clients in the Central & Southern Gold Coast.

This is a hands-on leadership role suited to someone who can manage risk, complexity, and people , not just coordinate.

Key Responsibilities

• Lead care planning and service delivery for clients with complex needs
• Manage and support a team of Support Workers (disability & child safety)
• Oversee incident management, reporting, and risk mitigation
• Ensure compliance with NDIS Practice Standards & Child Safety requirements
• Conduct client assessments and review care plans
• Provide staff supervision, coaching, and training
• Work with health professionals on complex care and behaviour support
• Participate in an after-hours on-call roster

About You

• Experience in disability services, child protection, or case management
• Strong understanding of NDIS, compliance, and incident management
• Confident leading teams in high-risk or complex environments
• Able to make decisions, manage competing priorities, and drive outcomes

Qualifications / Requirements:

• Degree in Psychology, Nursing, Social Work, Human Services or similar
• QLD Driver Licence
• Blue Card + NDIS Worker Screening Check
• First Aid & CPR
• LCS2 (or willingness to obtain)
