About the Role
As Manager (Care Planning & Practice), you’ll lead a team of Support Workers caring for our clients across Disability and Child Safety clients. Your role focuses on delivering exceptional care, ensuring compliance with regulatory frameworks, and fostering a positive team culture. You’ll work closely with health professionals and the Senior Manager to implement and evaluate care practices that meet our high standards.
Key Responsibilities
Provide professional care management advice and guidance to support staff.
Develop and implement clear care planning and practice frameworks, including individualized training.
Ensure accurate and timely incident reporting and compliance with NDIS Practice Standards.
Oversee behaviour recording and ensure staff understand client needs.
Collaborate with health professionals to design care regimens for clients with complex medical, mental health, or behavioural needs.
Conduct regular supervision and training of care staff and complete client needs assessments.
Participate in an after-hours roster to provide advice during critical incidents.
Ensure compliance with safety, HR quality frameworks, and financial protocols.
The Successful Candidate Will Have
Tertiary qualifications in Psychology, Nursing, Social Work, Human Services, Special Education, or similar.
Minimum 2 years’ experience in care/case management.
Knowledge of legislation and practice in at least one of the following: Disability Care, Forensic Disability, Child Protection, Aged Care, or Health Care.
Queensland Driver License.
Must hold or be willing to obtain: First Aid & CPR, Blue Card, NDIS Worker Screening Check, and LCS2 Background Check.
