About the Role
We are seeking a compassionate and motivated Social Worker to join our Centacare Post Release Service team. Working under the guidance of the Pastoral Ministries Director and Senior Social Worker, you will provide client triaging and referral, case management, advocacy and practical support to people transitioning from the criminal justice system back into the community. This rewarding role will see you working collaboratively with clients, government agencies and community organisations to help individuals overcome barriers and achieve sustainable outcomes. Key Responsibilities
- Provide person-centred case management and advocacy for clients.
- Develop, implement and review individual support plans.
- Conduct client assessments and coordinate referrals to appropriate services.
- Assist clients to access housing, employment, health, legal and financial supports.
- Build and maintain relationships with government agencies and community service providers.
- Advocate for clients to achieve positive and sustainable outcomes.
- Maintain accurate client records, case notes and reports.
- Ensure compliance with funding, legislative and organisational requirements.
- Contribute to continuous service improvement and quality outcomes.
About You To be successful in this role, you will have:
- A bachelor's degree or Diploma in Social Work, Social Welfare or a related discipline.
- Demonstrated experience in case management within the community services or welfare sector.
- A client-centred, strengths-based and solution-focused approach.
- Excellent interpersonal, communication, advocacy and relationship-building skills.
- Experience supporting vulnerable or marginalised individuals and navigating complex support systems.
- Strong organisational skills with the ability to manage competing priorities and work autonomously.
- Proficiency in Microsoft Office and electronic client management systems.
- A current Queensland C Class Driver Licence.
- A current Blue Card (or willingness to obtain).
- A current National Police Check (or willingness to obtain).
