Working within Centacare Services Aged Care and Home Safety, you will collaborate with a skilled multidisciplinary team, internal departments, and external agencies to coordinate and deliver responsive care. You will develop, implement, and review individualised care plans and service agreements, support clients in making informed decisions, and ensure services are delivered safely, effectively, and in line with Strengthened Aged Care Standards and Centacare’s governance and compliance framework.

About the role
As a Care Partner at Centacare, you will work directly with clients to identify their needs, goals, preferences, and existing supports, and assist them to make informed decisions about the aged care services they receive. You will develop, implement, and review individualised care plans and service agreements, coordinate and monitor service delivery, and ensure supports are provided in a flexible, responsive, and person-centred manner, in line with Strengthened Aged Care Standards.
You will operate within a robust governance and compliance framework, ensuring all services are delivered safely, effectively, and in accordance with Centacare’s Mission, Vision and Values, relevant legislation, and industry best practice. You will collaborate with internal teams, multidisciplinary professionals, and external agencies to maintain continuity of care and achieve positive client outcomes.
Key Responsibilities
- Conduct comprehensive assessments of clients’ needs, goals, preferences, and existing supports to inform care planning and service delivery.
- Develop, implement, and review individualised care plans and service agreements in accordance with Strengthened Aged Care Standards.
- Coordinate, monitor, and evaluate the delivery of services, ensuring flexible, responsive, and person-centred care that aligns with client goals and funding arrangements.
- Support clients, their carers, and Support Workers to make informed decisions, navigate aged care systems, and achieve wellness and reablement outcomes.
- Conduct risk assessments and proactively manage health, safety, and wellbeing risks for clients, staff, and other stakeholders.
- Collaborate with internal teams, multidisciplinary professionals, external agencies, and culturally appropriate organisations to maintain continuity of care and positive client outcomes.
- Participate in communities of practice, professional development, and quality improvement initiatives to embed best-practice approaches across aged care services.
What You’ll Bring - Relevant qualification in aged care, community services, nursing, or a related discipline, or equivalent demonstrated industry experience.
- Demonstrated experience in care planning, service coordination, and supporting clients in aged care or disability services.
- Knowledge of Support at Home, CHSP, and aged care funding standards, legislation, and regulatory requirements.
- Strong communication, stakeholder engagement, and report-writing skills.
- Ability to work independently, exercise professional judgement, and collaborate within a multidisciplinary team.
