Company/ Role:
We are looking for a motivated and compassionate Community Physiotherapist to join our dynamic Physiotherapy team. You will provide high-quality physiotherapy assessment and treatment to patients in their own homes, care homes, and community settings — helping people regain independence and improve quality of life.
Areas that we service include Sydney's Inner West & Eastern Suburbs.

Key Responsibilities

• Deliver evidence-based physiotherapy interventions in a variety of community settings.
• Assess, plan, implement, and evaluate individual treatment programmes.
• Work collaboratively with occupational therapists, nurses, social workers, and other healthcare professionals.
• Support hospital discharge and prevent avoidable admissions by promoting safe mobility and functional independence.
• Maintain accurate and timely clinical documentation.
• Contribute to service development and quality improvement initiatives.

About You

• AHPRA-registered Physiotherapist (BSc or MSc in Physiotherapy).
• Experience in community, rehabilitation, neuro or elderly care settings desirable. 
• Strong clinical reasoning, communication, and organisational skills.
• Ability to work autonomously and manage a varied caseload.
• Full driving licence and access to a vehicle (essential for community visits).
